Pituitary surgery – a modern approach

Surgery of the sellar region still represents, after a century from the first successful trans-sphenoidal approach, a superb and attractive challenge.

The anatomical complexity of this region, the effective and lasting recovery obtained in most sellar lesions – a very rare event in neurology – and the predictable and reliable results for any chosen therapy, account for this ‘success’.

The choice of therapeutic strategy for each lesion is, in some cases, extremely hard, as can be the follow-up of the patient. In fact, surgery often represents only one step in a long process starting with the formulation of the correct diagnosis and finishing, sometimes, with the set-up of a life-long replacement therapy.

To obtain the best result in this field, it is necessary to combine the efforts of different physicians, who are requested to work in team. This is the main reason for the creation of the ‘pituitary unit’ composed of endocrinologists, neurosurgeons, neuroradiologists, pathologists, radiosurgeons, and radiotherapists.

This book is the result of the long experience of one of the worldwide best recognized and appreciated pituitary unit. Practically all possible questions, different kinds of therapy and surgical strategies are investigated and analyzed. Very useful suggestions are given; most of the still controversial topics (e.g. treatment of Cushing’s disease failure) are examined thoroughly and up-to-date solutions are also illustrated. This feature is, above all, one of the key points of the volume. Its pragmatic structure is a real help for every physician involved in this field.

The basic necessity of working in team is ultimately one of the most important messages of this book. This is the only solution to obtain as many victories as possible against diseases of the sellar region.
